A LIFE CARE VISIT =
HOPE + RESOURCES.
We've served over 7 million cups of food & drink -- but our true measure of success is the impact we have on each person who is served a cup of soup.
And we are seeing significant increases in the number of people receiving long-term care.
A LIFE CARE VISIT is a one on one meeting with a trained outreach staff member in a safe & secure environment. Our staff makes an assessment and develops an individualized action plan for each guest, so our guests walk away with hope and a plan.
